You may submit a Notice of Claim if you are qualified for inclusion on the relevant voters list and believe that your information has been omitted or recorded incorrectly.

You can use this form if:

  • your name has been omitted from or wrongly stated on the voters list;
  • your address has been omitted from or wrongly stated on the voters list; or
  • your occupation has been omitted from, wrongly stated on, or has changed since it was recorded on the voters list.

You will need to state how you believe the voters list should be amended.

To complete the Notice of Claim, you will need to provide:

  • your full name;
  • your place of residence;
  • your occupation;
  • the electoral district for which you are claiming inclusion;
  • whether your claim relates to the provisional or preliminary voters list;
  • the information that has been omitted, incorrectly stated or changed; and
  • the correction you would like made to the voters list.

You must also sign and date the form.

When submitting a Notice of Claim, you must also complete the Applicant Personal Information Form.

Submit the Applicant Personal Information Form together with your Notice of Claim.

How to submit a claim

  1. Download and complete Form No. 3 – Notice of Claim.
  2. Select whether your claim relates to the provisional or preliminary list of voters.
  3. Indicate which information has been omitted, wrongly stated or changed.
  4. State how the voters list should be amended.
  5. Sign and date the form.
  6. Complete the Applicant Personal Information Form.
  7. Submit both completed forms to the Office of the Supervisor of Elections.

The Office of the Supervisor of Elections will review your claim and the amendment you have requested.

You may be contacted if additional information is needed to review your claim.
