Complete the Applicant Personal Information Form when submitting any of the following:

• Voter Registration Form
• Transfer of Registration Form
• Notice of Claim Form
• Notice of Objection Form

This is a supporting form and should be submitted together with the relevant Elections form.

The form asks you to provide your:

• full name;
• date of birth;
• father's first name;
• mother's maiden name;
• height;
• employment contact number;
• personal contact number;
• mailing address; and
• email address.

You will also be asked to provide identification information, as applicable, including your:

• Belonger's Card number;
• passport number;
• birth certificate number; and
• Social Security number.

The information requested is contained on the official Applicant Personal Information Form issued by the Office of the Supervisor of Elections.

1. Download the Applicant Personal Information Form.
2. Enter the date you are completing the form.
3. Provide your personal and contact information.
4. Enter the requested identification information.
5. Check that the information provided is accurate and complete.
6. Submit the form together with the relevant Elections form.

Do not submit the Applicant Personal Information Form by itself.

Submit it together with the applicable:

• Voter Registration Form;
• Transfer of Registration Form;
• Notice of Claim Form; or
• Notice of Objection Form.
